| Coalition Membership |
The coalition is comparable to a community steering committee made up of
members that represent a broad spectrum of the community. These members
help define health needs within the Izard County community, prioritize
those needs and participate in decision making to support action plans
to meet those needs. They also work to promote the mission, purpose, programs
and services of the coalition.
The membership consists of key representatives from various sections of
the population that impact factors of health. In order to preserve the
strengths of the coalition it it imperative that the membership be broad
based but not overpowered by a particular interest group. To ensure this,
the executive committee evaluates the membership on a yearly basis in October
in order to create a balanced membership of all sectors of the community
while maintaining a workable, functioning coalition. |
| Membership responsibilities include: |
- Knowing the coalition's mission, purpose, programs and services.
- Working to develop and implement strategies, goals, policies and procedures
that promote the coalition's mission.
- Fulfilling commitments made, willingly undertaking special assignments
and consider serving in leadership positions.
- Staying informed and following the trends which could impact the mission
of the coalition.
- Serving the community as a whole rather than any special interest group.
- Avoiding the appearance of conflict of interest that might negatively affect
the coalition. Disclose any possible conflicts to the coalition in a timely
fashion.
- Participation in a least one working committee.

| Executive Committee: |
The Executive Committee is the coordinating entity of Hometown Health Improvement
which consists of the coalition chairpersons, committee chairpersons and
health department support personnel. |
| Their duties include: |
- A leadership role in the community on behalf of the Izard County Hometown
Health Improvement Coalition.
- Fostering cooperation among members of the coalition.
- Coordination and facilitation of the activities of the coalition.
- Annual review of the coalition's membership to ensure appropriate representation
from the various sectors of the community.
- Evaluation of the coalition's progress.
- Facilitation of the planning process of the coalition as it works to meet
the mission and goals decided on by the membership.
- Decision-making should conflict or disputes arise within the coalition.
- Reviews projects to ensure compliance with coalition goals and submits
to coalition for approval.

| Standing Committees |
Standing Committees are formed based on the needs identified by the coalition.
These committees work to implement projects and strategies that promote
the coalition's mission and purpose. Committees must server the community
as a whole rather than any special interest group or constituency. They
are not limited in number, but are encouraged to follow the goals and objectives
identified by the assessment process. The committees may chose to form
sub-committees on an ad hock basis. Projects and recommendations for the
committee will be reviewed by the executive committee and coalition. |
| Community Support |
Hometown Health works to identify, support and facilitate existing organizations
and/or initiate the formation of new community partnerships, which promote
good health and well being in the community. |
| Funding |
The coalition does not currently have a 501c3 status, therefore the coalition
must rely on members and their organizations to act as the fiduciary of
any funds acquired by the coalition. The executive committee will exercise
prudence in decision making regarding this process |

Recommendations of the Executive Committee are presented to the entire
coalition. |
